Forms may be submitted by fax, mail, or in person. Most forms may be faxed directly to the Registrar's Office at 415-495-6756 unless otherwise instructed on the form. Some forms should be faxed to other offices depending on whether additional approvals are required before the office can process them. A few additional forms are available in hard copy at the office.
