This page is the official index of Golden Gate University forms. Forms are numbered by series so they are easy to reference: 100s for admissions and enrollment, 200s for records and registrar requests, 300s for tuition and financial aid, 400s for academic petitions, 500s for accessibility and support, and 600s for employees. If a copy of a form conflicts with the version linked here, use the version on this page.
Many requests no longer need a form at all. Registration, chosen name updates, and address changes happen directly in the myGGU portal through Self-Service.

Request a notarized credential or transcript. Complete and email to graduation@ggu.edu.
206 · Diploma or certificate replacement
Order an additional or replacement CeDiploma or CeCertificate through CeCredential Trust.
